Use this practical guide to organize your questions, paperwork, and arrival at Redding Family Medical Group. Start with the office to confirm current new-patient availability and the steps that apply to you.
1. Confirm how to get started.
Call the office before making plans around a first appointment. Ask whether the practice is currently accepting new patients for the care you need, which application steps come first, and how you will know the next step.
Downloading or completing a form is not an appointment booking or confirmation of acceptance. The application includes processing instructions; call for the current processing timeframe and approved return method before submitting it. Do not assume that an appointment can be scheduled immediately.
2. Check your specific insurance plan.
Have your current insurance card available. Give the office the full plan name and ask about participation in that particular network. An insurer’s name alone may not identify your plan.
Separately, ask your insurer about visit benefits, any copay, deductible or coinsurance, and whether a referral or authorization is required. If you have Medicare plus supplemental or Medicare Advantage coverage, have those details available too.

The website offers two printable downloads:
- New Patient Application: ask the office when to complete it, how to return it, and how the application process relates to scheduling.
- New Patient Paperwork: complete the pages requested by the office and ask when to bring or return them.
These PDFs are for downloading and printing. This website does not submit your application, save completed answers, or accept medical-document uploads. If you cannot open, print, or complete a form, call for assistance.
Do not email completed forms, insurance documents, or medical information through ordinary email. Ask the office for the appropriate return method. If a form refers to the practice’s Notice of Privacy Practices and you need a copy, ask the office; the website privacy page is not that notice.

- Photo ID and your current insurance card
- A current medication list
- Completed forms requested by the office
- Any referral or authorization required by your plan
- A short list of the questions you want to discuss
If you are moving from another practice, ask which records RFMG needs and how to arrange a transfer. If the visit is for a school, sports, employment, or DMV examination, mention that when scheduling and bring the requesting organization’s forms.
For a child’s appointment, ask about parent or guardian paperwork and attendance requirements. Call ahead if you need assistance preparing for or accessing the office.
5. Confirm where to go.
Our address is 2510 Airpark Drive, Suites 201 & 104, Redding, CA 96001. Confirm the correct suite for your appointment, especially if you have not visited before.
Office hours are Monday–Friday, 8 a.m.–6 p.m., with visits by appointment. Use the directions page to plan your trip, and call the office if you need help with arrival information.

No. Scheduling is by telephone. Call (530) 244-4034 to confirm availability and the steps to establish care.
Does sending an application guarantee an appointment?
No. Contact the office about current availability, application processing, and when an appointment can be arranged.
What if I cannot print the forms?
Call the office for help. Do not put personal or medical information into an ordinary email to request assistance.
