Using this website
This website provides general practice information. It does not collect appointment requests, accept payments, offer patient accounts, or ask you to submit medical information.
Please do not send personal health information, insurance documents, payment details, or completed medical forms through ordinary email. Call the office for help with the appropriate way to provide information.
Website technology
Website fonts and images are served with the site. We have not added advertising trackers or third-party analytics. The hosting service processes technical information needed to deliver the website, such as connection and request information.
This website is publicly accessible; no sign-in is required to view it. The hosting provider’s own privacy terms apply to that service.
Optional Google Maps
The map on the Visit Us page connects to Google only after you choose “Load Google Map.” Google may receive information such as your IP address, browser details, and map interactions, and may use cookies or similar technology under its own policies.
Directions links also take you to Google. You can choose not to use the map and call our office for assistance instead. See Google’s Privacy Policy (opens a new tab).
Patient forms
The patient forms are downloadable PDF files. This website does not provide a completed-form upload service. Bring completed forms to the office or call for instructions.
Health-information privacy
This website information is separate from the practice’s Notice of Privacy Practices, which explains how protected health information is handled. Contact the office to request the current Notice of Privacy Practices or ask a privacy question.
