Start with the form.
Schools, teams, and other organizations may supply their own examination forms. Have the form available when you call so you can explain the request, and bring it to the appointment.
Let the office know about any deadline so you can plan ahead. Ask what information to complete beforehand and whether additional records are needed.
Before the appointment
A little preparation helps.
Have your child’s insurance information ready, and ask about any required patient forms. Bring a current medication list and the paperwork requested by the office.
If records are held by another practice, ask which records RFMG needs and how to arrange their transfer. Ask about parent or guardian paperwork and attendance requirements when scheduling rather than assuming a school form covers those requirements.
